Background
Implementation of Safety Management System and Occupational Health in an enterprise is a requirement which is mentioned in the Act No. 13 of 2003 on Labor Article 87 that the company must implement a Safety Management System and Occupational Health Management System which is integrated with the Company.
These requirements are included in the investment firm for an obligation set forth in the Act the Government of the Republic of Indonesia.

1.         Hazard Identification, Assessment and Control Risk
Hazard identification carried out to determine the K3 implementation plan of the Company and the scope of work. Hazard identification is aimed at all sources, situation or activity that could potentially cause injury or occupational disease.
Hazard identification performed for all operational activities of the Company in the workplace include:
1.         Routine work activities and non-routine.
2.         Activities all those who entered the labor termpat including those around the work area, Sub-contractors, visitors and guests.
3.         Human culture, human capabilities and other human factors.
4.         Danger from outside of the workplace environment that can interfere with the safety and health of workers at work.
5.         Infrastructure, equipment and materials / materials in the workplace either provided by the Company and other parties related to the Company.
6.         Changes or proposed changes in the Company both changes in activity and substance / material / machinery used.
7.         Changes K3 Management System including temporary changes and their impact on operations, processes and work activities.

The risk assessment approach risk matrix method is relatively simple and easy to use, implement and present a visual representation in it.
Risk control is based on the following hierarchy:
1.         Elimination (eliminate the hazard).
1.         Substitution (replacing resources / equipment / machinery / ingredients / materials / activities / areas safer).
2.         Design (design / planning / modifications to the source installation / equipment / machinery / ingredients / materials / activities / areas in order to be safe) ,
3.         Administration (the application of procedures / rules of employment, training and visual control in the workplace).
4.         Personal Protective Equipment (provision of personal protective equipment for workers with exposure to the hazard / high risk).
Overall hazard identification, assessment and control of risk is documented and updated as the reference implementation of K3 in the Company.

2.         Regulation Legislation and Other Requirements
The company ensures compliance with laws and regulations and other requirements in the application of the Safety Management System and Occupational Health thoroughly. Laws and regulations may be laws issued by the government, a ministerial decree or K3 regulatory authorities regarding the company, which required licensing and other regulations issued by official government agencies. While other requirements may be a contractual requirement and cooperation, as well as other agreements with third parties.
The company guarantees the legislation and other requirements which are used can be easily identified, evaluated for compliance, accessible and are of legislation and other requirements that apply the latest and valid.

B.     APPLICATION
1.         Resources, Roles, Functions, Powers and Responsibilities.
To ensure the implementation of K3 in the workplace, the Company formed a Committee of Safety and Health (P2K3) in accordance with the Regulation of the Minister of Manpower No. 4 of 1987 of the Committee of Occupational Safety and Health (P2K3).
Committee of Occupational Safety and Health (P2K3) is the definition of a subsidiary body in the workplace which is a forum of cooperation between employers and labor to develop mutual understanding and cooperation in the implementation of K3 effective participation in the Company.
The composition of the P2K3
Task Committee of Occupational Safety and Health is to provide advice and whether requested or not by Entrepreneur / Executive Board regarding occupational health and safety issues within the Company.
Function Committee of Occupational Safety and Health, among others:
5.         Collecting and processing data regarding Occupational Health and Safety (K3) in the workplace.
6.         Help demonstrate and explain to all workers regarding:
a.         Various factors in the workplace hazards that can cause interference K3 including fire and explosion hazards, and how to mitigate them.

c.         Personal Protective Equipment (PPE) for workers concerned.
d.         Ways and secure the right attitude and do the job.
7.         Helping Employers / Management in:
a.         Evaluate how, processes and work environment.
b.         Determine the corrective action with the best alternative.
c.         Develop hazard control system of the Occupational Health and Safety.
d.         Evaluating the causes of accidents, occupational diseases (PAK) as well as taking the necessary steps.
e.         Develop education and research in the field of occupational safety, hygiene company, occupational health and ergonomics.

The company ensures fulfillment of the relevant resources for the implementation of occupational safety and health in the enterprise environment.

2.         Feasibility, Training and Knowledge.
The company guarantees the feasibility of all the personnel who have to carry out the implementation of the Safety Management System and Occupational Health in the Code in accordance with the background, skills, training and experience of the personnel respectively.
The company is also carrying out the identification of training needs for all personnel under the control of the Company based on competence, expertise and hazards associated position and the type of work in order to ensure the implementation and application of the Safety Management System and Occupational Health are both within the company.

3.         Communication, participation and consultation.
3.1.Communication
14.     To ensure the implementation of Safety Management Systems and Health at Work, the Company prepares a communication system to support the implementation of Safety Management System and Occupational Health are both in the workplace. Communication includes internal communication within the organizational structure of the Company as well as external communication with other parties such as those around the work area, Sub-contractors, visitors, guests and the public and third parties who work together with the Company with respect to K3 (Occupational Health and Safety ).
Communication can be via a variety of media, how and technology that can effectively deliver a message to all those who need to receive information relating to the Implementation of Safety Management System and Occupational Health.

3.2.        Participation and Consultation.

5.         Control Document.
All documentation and information used in the implementation of Safety Management System and Occupational Health should be identified and controlled. Control of documentation including approval procedures is about the document, publication, storage and destruction of documents.
All documents and documentation will be available when needed in an operational condition of routine and non-routine, including when a state of emergency. All documentation will be published in an official document list managed by the Secretary of the Committee of Occupational Health and Safety, including information regarding the authority of the approval document, publication, dissemination, revision, location, storage and destruction of documents.

6.         Operations Control.
Once all K3 workplace hazards have been identified and understood, the Company adopted the necessary operating controls to manage the risks associated K3 hazards in the workplace and to comply with laws and regulations and other requirements associated with the implementation of K3 in the workplace.

2.         Internal audit
